OVR Transition Coordinator

A company is looking for a Transition Coordinator to support students and participants with disabilities in preparing for employment and independence. Key Responsibilities Work with adolescents aged 14-21 to develop and implement transition programming Coordinate career exploration, job readiness, and community-based experiences Serve as a liaison between programs and external partners, maintaining documentation and compliance Required Qualifications, Training, and Education Experience working with individuals with disabilities Knowledge of vocational rehabilitation processes and resources Ability to collaborate with educators and community agencies Familiarity with career exploration and job readiness programs Must be based in the Hampden, PA area for this remote role
