7300 PharmacySchedule: Night Shift.Wed - Thur 7p-6aFri - Sun 7p-7a,Mon - Tue 7p-6aAbout The PositionThe Pharmacist is responsible for the safe and effective use of medication throughout the health system.Overview Of ResponsibilitiesReviews and interprets provider orders and performs clinical checkingCommunicates clinical concerns such as allergies, drug interactions, side effects, and min/max warnings to providersMonitors patient medication therapies and provides interventionsApproves all medications and IV’s before they are dispensed.Counsels patients and caregivers on medication use.Works proficiently in all pharmacy information systemsPrepares medications during Code Blue SituationParticipates in adverse drug reaction review and reportingDocuments all clinical activitiesPerforms thorough technician evaluationsParticipates in QA&I program or other related projectsParticipates in antimicrobial stewardship activities (if applicable)Provides thorough shift to shift communicationReports medication errors, prevented and actualThis list of duties and responsibilities is not intended to be all-inclusive and can be expanded to include other duties or responsibilities that management deems necessary.Education & ExperienceMinimum Education Required: Bachelor of Pharmacy degreeMinimumEducation Preferred: Pharm D degree from an ACPE accredited college of pharmacyLicensure & CertificationActive pharmacist license in the State of Indiana.
