Location is dependent on office availability.\nArea of Focus: Behavioral Health\nProgram(s): PATH - Projects for Assistance in Transition from Homelessness helps adults experiencing homelessness or marginal housing and those living with significant mental illness transition to more stable housing by coordinating services that help them address their individual needs.\nCase Manager Immediate Supervisor: Community Transitions Director - Daniel Hyson\nCase Manager Hours of Work: Full-time, 40 hours/week.
