This individual serves as a primary operational contact for partner relationships, referral workflows, and daily execution to ensure high-quality, patient-centered care. Working closely with Care Navigation, Clinical, Finance, and Technical teams, the Operations Coordinator supports operational execution, assists with data-informed tracking, and helps identify opportunities to improve partner responsiveness, workflow efficiency, and caregiver experience.\nThis role sits at the intersection of operations, data, finance, contracting, and technical collaboration and requires strong attention to detail, proactive communication, and the ability to independently manage day-to-day partner workflows while working closely with internal care teams.\nThe Day-to-Day\nPartner Sourcing & Contracting\nAssist with sourcing and evaluating new RCC, respite care, and franchise partners to expand geographic coverage and patient access.\nCoordinate partner onboarding by collecting required documentation, credentialing materials, and compliance agreements.\nMaintain accurate partner records, service areas, and operational updates within internal systems and the CMS Portal.\nTrack contract renewals, amendments, and expirations to ensure uninterrupted service coverage across the network.\nVendor & Franchise Relationship Management\nServe as the primary daily contact for RCCs, respite vendors, and franchise locations to address routine inquiries and support needs.\nManage daily referral workflows, ensuring clear communication and timely escalation of urgent or complex cases.\nConduct routine outreach and check-ins with partners to maintain engagement, monitor service standards, and review performance.\nCollaborate with Care Navigation and partner organizations to troubleshoot and resolve daily barriers to care.\nMaintain accurate documentation of partner interactions, operational issues, and follow-up actions.\nInternal Collaboration & Process Improvement\nPartner closely with Care Navigation, Finance, and Technical teams to streamline referral workflows and resolve billing or payment queries.\nHelp draft, update, and maintain standard operating procedures (SOPs), workflow guides, and escalation pathways.\nIdentify recurring operational bottlenecks and suggest practical process updates to improve efficiency and caregiver\nexperience.\nSupport the rollout and team execution of new operational initiatives and system enhancements.\nData & Analytics\nTrack key operational metrics, including referral volumes, turnaround times, utilization, and partner responsiveness.\nPrepare and distribute routine performance reports and basic dashboards for leadership review.\nPerform regular reviews of partner, operational, and billing data to ensure accuracy, integrity, and compliance.\nMinimum Qualifications\n2–3+ years of experience in operations, healthcare operations, care coordination, vendor support, or network administration.\nExperience working with external partners, vendors, or providers (healthcare or social support setting preferred).\nStrong attention to detail with the ability to track and manage multiple operational tasks simultaneously.\nComfort working with spreadsheets (Excel/Google Sheets), basic dashboards, and system portals.\nStrong written and verbal communication skills.\nProven ability to work cross-functionally and manage daily tasks in a remote environment.\nPreferred Qualifications\nExperience or familiarity with respite care, residential care/skilled nursing care, home health, or caregiving services.\nBasic familiarity with contract coordination, administrative compliance, or credentialing processes.\nCompensation\nFinal compensation will be determined based on location, experience, and qualifications.\nOur Values\nWe are a remote-first company.
