This role conducts in-depth assessments of each person’s needs, develops individualized service strategies, and ensures that appropriate resources and supports are in place.\nThe Program Specialist I coordinates services across multiple areas including medical care, housing, social services, employment or day programs, recreation, and community engagement. The role focuses on developing, implementing, and monitoring Individual Support Plans (ISPs) to ensure the health, safety, and overall well-being of the individuals supported.\nThe Program Specialist works collaboratively with staff, families, community providers, and other stakeholders to ensure individuals receive comprehensive services and opportunities to thrive.\nSalary: $54,999.88\nMajor Responsibilities\nProgram Oversight and Support\nEnsure residential services are delivered in a professional, respectful, and person-centered manner.\nEnsure services comply with all applicable agency policies, licensing standards, and regulatory requirements.\nMonitor program operations and evaluate staff performance related to care delivery.\nEnsure adequate program supplies, equipment, and resources are available.\nMake recommendations for program improvements when necessary.\nWork closely with admissions and discharge teams to identify individuals needing supportive living services.\nCase Management Coordination\nEnsure individuals supported receive adequate and appropriate services.\nCoordinate care among internal staff and external providers.\nMaintain accurate and confidential records for each person supported.\nCommunicate with healthcare professionals and service providers as needed.\nConsult with family members, guardians, and other support networks to ensure continuity of care.\nIndividual Support and Home Care\nProvide person-centered support in the home and community.\nAssist in identifying and managing home care needs, including:\nPersonal hygiene\nHousehold management\nDaily living activities\nRespond to unexpected situations or changing support needs.\nMedical and Service Coordination\nCoordinate and schedule medical appointments and related services.\nEnsure medical recommendations and treatment plans are followed.\nCommunicate medical instructions and updates to staff and caregivers.\nCommunity Resource Coordination\nEstablish and maintain relationships with community service providers.\nAssist individuals in accessing community resources and services.\nEducate individuals, families, and support systems about available program services and community supports.\nAgency Liaison Responsibilities\nServe as a liaison between individuals supported, staff, families, guardians, and external service providers.\nMaintain strong communication between all parties involved in the support plan.\nEnsure concerns and service needs are addressed promptly.\nAdministrative Responsibilities\nMaintain confidential files and documentation for individuals supported.\nPrepare reports and documentation as required by the agency.\nParticipate in meetings, committees, and program planning activities.\nAssist with resource allocation and program support.\nPerform additional duties as assigned.\nQualifications\nEducation and Experience\nHigh School Diploma or equivalent required.\nBachelor’s degree from an accredited college or university preferred.\n3–5 years of experience working in the disabilities field or related human services setting.\nCore Competencies\nStrong organizational and time-management skills.\nDetail-oriented with strong documentation abilities.\nAbility to work independently with minimal supervision.\nStrong problem-solving and decision-making skills.\nCollaborative mindset and ability to work effectively with teams.\nAbility to develop and implement new services or procedures when needed.\nStrong interpersonal and communication skills.\nAdditional Requirements\nEmployees must:\nMaintain compliance with OSHA, HIPAA, and agency safety policies.\nAttend required trainings and meetings.\nMaintain accurate documentation of time worked and assigned tasks.\nFollow all organizational policies and procedures.\nEquipment and Technology\nComputer and Microsoft Office software\nTelephone and mobile communication devices\nAgency documentation and reporting systems\nProgram vehicles and adaptive equipment when necessary.\nWorking Conditions and Physical Requirements\nEmployees must be able to:\nLift up to 50 pounds.\nSit, stand, walk, bend, and reach during the workday.\nDrive to program locations and participant homes.\nConduct visits in community settings and residential homes.\nWork environments may include:\nOffice settings\nResidential homes\nCommunity locations\nOutdoor environments.\nSafety and Compliance Knowledge\nEmployees must maintain awareness of and compliance with:\nBloodborne Pathogen standards\nOSHA safety guidelines\nHIPAA confidentiality regulations\nFire and safety procedures\nRight-to-Know regulations.\n\nAbout Company:\nApis Services, Inc.
