If you're a Physical Therapist who values meaningful work, work-life balance, and professional growth, we encourage you to apply.Required QualificationsMasters degree or higher in Physical TherapyActive and unrestricted Physical Therapist (PT) license in the State of IllinoisMinimum of 1 year of experience in a healthcare settingValid drivers license and ability to travel locallyPreferred QualificationsPT license number provided at time of applicationKey ResponsibilitiesConduct comprehensive physical therapy evaluations and develop individualized treatment plansProvide in-home therapy services focused on improving physical and cognitive functioningMonitor, document, and report patient responses to treatment and changes in conditionEducate patients and caregivers on therapeutic exercises, safety, and use of adaptive equipmentSupervise Physical Therapy Assistants and, when appropriate, Certified Nursing AssistantsComplete required documentation, prepare clinical reports, and participate in team and agency development activitiesSchedule & TravelMonday–Friday | Day shift (start and end times vary based on patient needs)One weekend every six weeks (Saturday & Sunday, scheduled visits only)Two holidays per year (one summer, one winter on a rotating basis)No on-call requirementsLocal travel to residential homes
