We are committed to breaking the cycles of poverty and exclusion that impact so many in our city.\n\nPosition Summary\n\nThe HOPE Program is seeking a Case Manager who will engage with training participants and alumni in both one-on-one as well as groups setting to provide counseling, guidance, and referral, and case management services to a case load of 120 to 150 individuals, including both low and high touch cases. The position receives daily supervision from the Senior Case Manager and reports to the Director of Client Engagement. The Client Engagement team as a whole is responsible for meeting the overall recruitment, admissions, and case management goals resulting in enrollment, completion, placement, and retention outcomes in line with HOPE’s strategic plan. In doing so, the team will assess training candidates for proper fit in order to commit to HOPE’s full-time training program as well as provide wrap-around services, wellness support and case management to address barriers that may impede their ability to successfully complete training and secure employment. The Case Manager is expected to work a minimum of four days from the office across all HOPE and partner locations and work schedule will include evening and weekend hours.\n\nResponsibilities\n\nProvide comprehensive, participant-centered case management services to individuals experiencing barriers to employment and self-sufficiency, including participants in training, participants in job search phase, and alumni.\nMaintain regular contact with participants through individual meetings, phone calls, virtual appointments, and classroom visits to monitor progress and provide ongoing support.\nUtilize trauma-informed, strength-based, and motivational interviewing techniques to promote participant engagement, empowerment, and goal attainment.\nProvide crisis intervention, conflict resolution, and supportive counseling while responding appropriately to participant needs.\nConnect participants to community resources and supportive services, including housing, public benefits, healthcare, mental health services, childcare, legal assistance, food resources, and workforce development programs.\nMaintain current knowledge of community resources, New York City workforce systems, HRA, and community-based organizations.\nContribute to HOPE’s working database of social service partners and resources in areas, such as public benefits, affordable housing, childcare, pro bono legal services, basic needs access, etc.\nMaintain and cultivate partnerships for social service referrals for students that are warm and human-centered, recognize the needs of participants and capacity of the provider, and offer a feedback loop to HOPE.\nCollaborate with instructors, employment specialists, recruitment staff, and other program personnel to support participant retention and successful employment outcomes.\nFacilitate workshops, orientations, and group discussions related to wellness, life skills, employment readiness, community resources, and other participant development topics.\nMaintain accurate, timely, and confidential case notes, assessments, service plans, referrals, and participant records in HOPE’s Salesforce data management system.\nParticipate in participant conferences, staff meetings, and program planning discussions.\nDemonstrate professionalism, cultural humility, and ethical practice when working with diverse participant populations.\nManage a diverse caseload while prioritizing multiple responsibilities and meeting program deadlines.\nTravel weekly between program sites and community partner locations, as required.\nAssist with intakes as needed and perform other duties as assigned that are consistent with the responsibilities of the Case Manager position.\n\nQualifications/Requirements\n\nThe following are the ideal qualifications for this position. A successful candidate will possess a compelling combination of strengths in some areas and the self-awareness to identify areas where they are eager to learn and grow.\n\nBachelor’s degree in Social Work, Psychology, Counseling, Human Services, or comparable educational background or training.\nCandidates pursuing a related degree are encouraged to apply.\nExperience in case management or supportive services.\nKnowledge of trauma-informed care and New York City resources and services available to support participants.\nStrong communication, organization, and time management skills.\nExperience facilitating workshops or group sessions.\nProficiency in Google Workspace.\nAbility to travel between program sites.\nProfessionalism, cultural humility, and a commitment to supporting diverse populations.\n\nWork Environment\n\nThis is a hybrid position working at least 4 days per week at HOPE and partner locations and work schedule will include evening and weekend hours.\nWork hours for this position may be subject to change based on organizational needs. Employees may be required to adjust their schedules, including working outside of standard business hours, as necessary to fulfill the responsibilities of the role.\nEmployees serve as ambassadors of The HOPE Program and may be called upon to represent the organization at community events, partner meetings, public forums, and other external engagements, helping to advance our mission and strengthen relationships with stakeholders, including employers and community partners.\n\nSalary and Benefits\n\nThe annual salary range for this position is $50,000 - $60,000, and is dependent upon experience.
