Notifies the clinical team of any changes in resident physical and/or mental status.Provides personal care to residents per their individualized plan of care.Completes prevention and restorative care including, but not limited to, ambulation, transfers, bathing, grooming, and all other ADL care.Documents all pertinent information regarding care as assigned.Assists in turning, positioning, and repositioning of residents as outlined in the resident's service plan.Communicates effectively with the residents, families, visitors, members of the health care team, and other disciplinesRequirements:NJ Certified Home Health Aide (CHHA) certification must be current and activeAbility to work in a collaborative work environment while being accountable for individual goalsAbility to respond to and work with a mature population, contributing to a warm, caring and supportive environment for the enjoyment of the residents/clients and to promote this philosophy to all team membersMust be available to work weekends, as scheduledBenefits:401(k)Dental insuranceFlexible scheduleFlexible spending accountHealth insurancePaid time offReferral programVision insuranceWork Location: In personEOE: FellowshipLIFE is an equal opportunity employer.
