QIDP (Case Manager) Bachelors Degree & DD Experience Required

Goldie Floberg QIDP's do not work as DSPs or Supervise DSPs. On-call hours are not covering DSP hours, only medical emergencies that can't be covered with DSPs. At Goldie Floberg Center, our QIDPs play a key role in supporting individuals with developmental disabilities to achieve their goals, build independence, and thrive in their communities.  The QIDP role is a great position to gain case management experience for recent graduates! What You’ll Do: As a QIDP, you’ll partner directly with the people we serve, their families, and support teams to create and guide individualized service plans. You’ll lead Community Support Team (CST) meetings, coordinate care, and ensure services align with each person’s goals and preferences. This role blends office work with hands-on time in homes and the community—making a real impact every day. What Makes This Role Unique: Meaningful, relationship-driven work  A mix of office and field-based responsibilities  Flex scheduling with opportunities for remote work  Direct involvement in improving quality of life for others  Schedule & Expectations: Full-time (minimum80 hoursper pay period)  Typical hours:8:30 a.m. –5:00 p.m. with flexibility  Approximately 25% of time spent in the field  Somepre-plannedevening/weekend hours and rotating on-call responsibilities  What You’ll Need: Bachelor’s degree in Social Work, Psychology, Special Education, or related field  At least 1 year of experience supporting individuals with intellectual/developmental disabilities  Eligibility for (or current listing on) the DHS/DDD QIDP Registry  Strong communication, organization, and multitasking skills  Valid driver’s licensewith clean recordand ability to travel locally  Why Goldie Floberg? We are mission-driven and committed to empowering and enriching the lives of the people we serve.
