We accomplish this mission through various social, educational, and recreational services that reach community members of all ages and backgrounds.\n\nSTATEMENT OF THE POSITION:\nWithin our framework of a holistic, strength-focused, and family-supported practice, an individual hired as the Program Coordinator (Case Manager), Learn & Earn Site Director will report directly to the Site Director, Learn & Earn Program. Office will be based out of the Queens High School Teaching.\n\nEDUCATION & QUALIFICATIONS\nMSW or related Masters Level Degree, SIFI Certification required or eligible.\nAt least two (2) years’ experience working with adolescents in an urban environment, providing services that reduce barriers and facilitate educational success and social-emotional well-being.\nAt least two (2) years of experience providing outcomes-oriented supervision and professional development to a team of youth development specialists.\nExperience working with youth in a school setting.\nGood supervisory skills and ability to work as part of a team.\nConflict resolution and negotiating skill.\nComputer literate, including experience with,Google Suite and Microsoft Office software packages, local area networks, and the Internet.\nStrong organizational skills.\nExcellent written and oral communication skills.\nAbility to engage in productive supervision.\n\nDUTIES AND RESPONSIBILITIES:\nSupport in student recruitment; assist in conducting comprehensive intake and assessment to determine program eligibility and develop an individual service strategy for eligible participants. Complete the required documentation and submission via funder mandated portal (DYCD Connect).\nEstablish a rapport with all program participants. Assist them in identifying their educational and career goals; help them to formulate future plans.\nImplement and conduct weekly workshops on work readiness, career development, college readiness, life skills, service learning, and student enrichment.\nMonitor the attendance, academic, and career progress of program participants.\nProvide and structure a comprehensive interpersonal group and individual counseling program to meet the varying needs of students.\nWork in a collaborative team, including participation in staff meetings, contractor meetings, and staff development in order to support student progress.\nMonitor and document student progress in various service areas (guidance and counseling, academic support, life skills, work readiness).\nConduct daily attendance and participation outreach.\nParticipate in ongoing (monthly) professional case conferences, schedule site-based training as well as staff meetings, with agendas and attendance sheets.\nSupport in cultivating competitive Summer Internship Program placements for contract cohort that align with identified career goals and objective.\nMonitor the completion and submission of monthly case notes on the tenth (10th) of the month for the previous month for current and follow-up participants.\nSubmission of contractual milestones including: Measurable Skills Gain, Attainment of HS Diploma and quarterly placement during follow up year.\nPartnering to support and ensure success of all Commonpoint programming on-site at the Queens High School of Teaching.\nSupervision and mentorship for Social Work Interns within the Learn & Earn Program.\nConduct regular visits to participant work sites to provide participant support and to keep track of engagement.\nProvide necessary referrals and connections to community resources.\n\nGeneral Tasks of All Commonpoint Staff:\nPromote and implement the core values of the agency.\nParticipation in all staff meetings and staff training seminars.\nParticipation in agency-wide programs and program committees, as assigned.\nActive involvement, as a supervisee, in the supervision process.\nSupport the mission of Commonpoint.\nOther tasks appropriate to Commonpoint staff, as assigned.
