Job Summary/Objective:Assure the highest quality of care for Chateau residents, within the scope of practice, by assessing, analyzing, monitoring, and treating their medical and psychological needs, while also providing the highest quality of customer service to residents,as well as their family members and support system. Provide direct supervision to CNAs and CMAs who provide health care services in the Chateau. Supervisory Responsibilities:Lead and manage direct reports to assure the desired level of resident service, care, and satisfaction. Monitor and evaluate employee performance; provide coaching, training, and corrective actions as required. Assure equitable administration of policies, and compliance with regulatory and legal requirements. Essential Functions/Duties/Responsibilities:Regularly assess and chart the resident's physical condition, monitoring for any significant change and generate appropriate internal and/or external response. Communicate medical data to physicians regarding resident's condition. Collaborate with medical team, note physician's orders, and coordinate with appropriate pharmacy and/or ancillary department staff to assure follow through with orders for testing, medication, or treatment as ordered. Administer medications and treatments that have been ordered by the resident's physician (i.e. finger stick blood sugars, insulin, tube feedings, IV therapy, nebulizer, etc.) to assure the highest level of care for the resident and document all medications and treatments appropriately. Document and complete monthly resident reports, transfer records, and weekly skin reports per Montereau policy and procedure, to assure a high quality of care is maintained and service documentation is maintained for billing accuracy. Monitor resident charts for activity in a 24-hour period to assure that all orders and medical treatments are properly input into the resident's chart to prevent any medication errors. Monitor residents in the dining room during meals to assess and resolve problems and monitor for choking, allergic reactions, cognitive impairment, or change, etc. Assist Certified Nurse Aides (CNAs) with personal care for the residents to assure that a high level of quality care is routinely provided. Prepare shift change reports for oncoming shift and provide a daily briefing to assure a clear transition between shifts in order to maintain continuity of care. Teach and demonstrate medical techniques to residents, residents' families, staff, and students, when applicable, to assure maximum effectiveness and continuity of care during a resident's time with Montereau and following discharge. Immediately report any incident of resident abuse or neglect, or suspected resident abuse or neglect, to appropriate health services leadership. Run calibration checks with glucometers to assure that equipment is functioning properly, and the readings are accurate. Monitor and record the temperature of the refrigerator and medication room to assure that medications are being stored properly and ready for use as needed. Participate in weekly Care Conferences, as needed. Attend required nursing in-services and Extended Leadership Team (ELT) meetings and complete all required educational online training programs. Provide administrative support to the unit to assure a high level of effective operation of the unit. Activities may include: answering the unit phone, faxing and mailing appropriate documents, communicating supply needs to the supply clerk, scheduling resident appointments with physicians, arranging transportation, etc. Monitor and mentor clinical nursing and nurse aide students' capabilities, assist with the learning process and oversee and assess their interaction with residents. Review student progress with instructors and monitor instructor involvement while shadowing their students within Montereau. Perform other related functions as required or as assigned. Education and Experience Previous skilled nursing experience within a long-term care (LTC), assisted living, Continuing Care Retirement Community (CCRC), or hospital environment is preferred Previous leadership experience in a health care setting is preferred Current State of Oklahoma Registered Nurse license is required Current CPR certification is required Thorough knowledge and understanding of the Nurse Practice Act, Rules, and Regulations Strong knowledge of methods for administering medication, wound dressing, IV therapy, tube feedings, and flushes Working knowledge of Microsoft Office programs and ability to learn new software systems (i.e., PCC, AOD, OnShift, Outlook, etc.) Strong communication skills to effectively and empathetically address critical health and end of life issues with a diverse resident population and family members Strong interpersonal skills and understanding of quality customer management and service Ability to effectively manage sensitive and confidential communications and information, while maintaining a high level of poise, tact and diplomacy Solid analytical ability and problem-solving skills Ability to effectively interact with a diverse leadership team, staff, and third-party associates, including vendors, providers, and visitors Physical Requirements: An extensive amount of walking, standing, and sitting, with a moderate amount of bending at the waist, kneeling, twisting/turning, reaching above the head and shoulders, and eye-hand coordination Ability to routinely lift 25 pounds and push wheelchairs with loads of up to 300 pounds Frequent use of ordinary communication, including normal levels of hearing and vision, including both near and far seeing, color discrimination, field of vision, and depth perception Other Duties: Please note this job description is not designed to cover or contain a comprehensive listing of activities, duties or responsibilities that are required of the employee for this job.
