The Case Worker will provide social services support to patients and families, assisting with care coordination, resource placement, and eligibility determination for various programs.Responsibilities• Provide social services to patients, including those requiring public assistance or institutional care• Conduct assessments to determine patient needs and eligibility for services• Develop and implement individualized service plans• Assist with placement in nursing homes and long-term care facilities• Coordinate referrals and case management activities• Collaborate with healthcare teams to support patient care plans• Provide counseling and support to patients and families• Maintain accurate documentation and case records• Utilize systems such as EPIC and Careport for referrals and coordinationRequirements• Bachelor's Degree from an accredited college or university• Minimum 3 years of case work or social services experience• Experience with nursing home placement and care coordination• Strong communication and interpersonal skills• Ability to manage multiple cases effectivelyPreferred Qualifications• Experience with EPIC and Careport systems• Background in hospital or healthcare settings• Strong knowledge of community resources and social service programsPride Health offers eligible employees comprehensive healthcare coverage (medical, dental, and vision plans), supplemental coverage (accident insurance, critical illness insurance, and hospital indemnity), 401(k) retirement savings, life & disability insurance, an employee assistance program, legal support, auto and home insurance, pet insurance, and employee discounts with preferred vendors.
