Social Care Navigator
A company is looking for a Social Care Navigator (Remote).
Key Responsibilities
Conduct high-volume outreach and screening for Medicaid members to assess eligibility and identify next steps
Utilize a virtual contact center platform to manage member interactions and document screening outcomes
Provide accurate information and navigate members to eligible nutrition services and support resources
Qualifications
High School Diploma, HSE, or GED required
Experience in high-volume outbound calls, customer service, or call center environments
Working knowledge of Medicaid and healthcare benefits preferred
Familiarity with community resources and social service programs preferred
Proficiency in operating computer software and technology platforms for documentation
