Responsibilities include providing discharge planning guidance, offering utilization management support, and leveraging clinical nursing expertise for comprehensive case reviews. This is a full-time, on-site position located at Lehman College SCPS in Bronx, NY.QualificationsStrong background in Nursing and Medicine to provide professional and educational guidanceExperience in Case Management and Discharge Planning for customized resource allocation and client supportProficiency in Utilization Management to assess and ensure appropriate use of available resourcesStrong organizational, communication, and interpersonal skillsActive RN license and familiarity with NCLEX exam requirementsBachelor’s degree in Nursing or a related field is required; advanced degree is a plusExperience in educational coaching or mentoring is an advantage
