This role focuses on assessing patient needs, improving health outcomes, and ensuring access to appropriate clinical and social resources through a collaborative, team-based approach.Key ResponsibilitiesAssess members’ clinical, psychosocial, financial, and environmental needsDevelop and implement individualized care plans in collaboration with members, caregivers, and providersSupport members in managing chronic conditions and improving overall healthCoordinate care with primary care physicians, specialists, and interdisciplinary teamsIdentify and address gaps in care, including social determinants of healthConnect members to community resources and support servicesProvide transition of care management to reduce hospital readmissionsEducate members on chronic disease management, preventive care, medication adherence, and home safetyAdvocate for members to support informed decision-making and improved outcomesPerform additional duties as assignedRequired QualificationsActive RN license in New York (open to obtaining additional tri-state licenses)Minimum 2 years of care management experienceHealth plan experienceStrong communication, teamwork, and independent work skillsAbility to think critically and problem-solve in a fast-paced environment
