If you are passionate about making a positive impact in patients' lives and working in a supportive and family-oriented atmosphere, we encourage you to join us in providing exceptional home health care.Please note that this role is per diem with a rate of $180 - $200 per visit.ResponsibilitiesEvaluate patients' functional status, mobility, muscle strength, gait, and ROM to develop individualized treatment plansDeliver physical therapy services in accordance with the physician's plan of care and California's Physical Therapy Practice ActEducate patients and their families or caregivers on therapeutic exercises, equipment use, and recovery goalsCollaborate with physicians and the care team—reporting changes in patient condition and participating in case conferencesSupervise Physical Therapy Assistants and, when PT is the sole skilled service, oversee Home Health Aide care bi-weeklyManage documentation including clinical progress summaries and discharge planningIdentify needs for additional home health services and coordinate referrals as appropriatePerform other duties as required for patient servicesRequirementsBachelor's or Master's degree in Physical Therapy from an accredited programActive California Physical Therapy licenseMinimum 2 years of PT experience; home health background is a plusStrong communication skills and the ability to connect with patients and familiesCurrent CPR certification with AHAValid driver's license and reliable, insured vehicleBenefitsFlexible ScheduleAbility to Convert to Full-Time Based On Performance If Interested Paid Sick Days Paid Bereavement LeaveSkills Training & DevelopmentCompany Social EventsMileage Reimbursement for Patient to Patient VisitsReferral Bonus
