This allows for continuity and more thoughtful, relationship-based care across settings.The JobAs an Advanced Practice Provider - Nurse Practitioner (NP) or Physician Assistant (PA) - at Welcome Health, you will provide primary care for older adults as part of a physician-led, multidisciplinary team. This role primarily supports the Bell clinic and focuses on high-quality longitudinal care for patients with a range of medical and social needs.This is a primary care role with the opportunity to incorporate cardiovascular disease management. The ideal candidate is comfortable managing a primary care panel while also supporting patients with cardiovascular conditions in collaboration with cardiology partners.
