success.\r\nEssential ResponsibilitiesParticipant Assessment and Service PlanningAssess participant strengths, needs, barriers, and goals.\r\nIdentify appropriate services, resources, and supports based on individual needs.\r\nDevelop, implement, and monitor individualized service and treatment plans.\r\nSupport participants in achieving recovery, wellness, and self-sufficiency goals.\r\nConduct screenings and referrals to appropriate programs and providers.\r\nCase Management and Care CoordinationCoordinate services among healthcare providers, behavioral health professionals, community agencies, and support systems.\r\nFacilitate access to treatment, housing, employment services, transportation, and other community resources.\r\nAssist participants with obtaining benefits and entitlements, including Medicaid and other public assistance programs.\r\nMonitor participant progress and adjust service plans as needed.\r\nCollaborate with participants, families, and service providers to ensure continuity of care.\r\nAdvocacy and Resource NavigationAdvocate on behalf of participants to remove barriers and improve access to services.\r\nHelp participants navigate complex healthcare, behavioral health, and social service systems.\r\nConnect individuals with community resources that support long-term recovery and stability.\r\nPromote participant self-advocacy and empowerment.\r\nAdditional ResponsibilitiesParticipate in multidisciplinary team meetings and care planning discussions.\r\nBuild and maintain positive relationships with community agencies and referral partners.\r\nIdentify service gaps and provide recommendations for program improvements.\r\nComplete all required assessments, case notes, documentation, and compliance records.\r\nCoordinate services across internal programs and external agencies.\r\nEnsure compliance with all funding, licensing, accreditation, and organizational requirements.\r\nSupport program operations and administrative case management activities.\r\nAttend required trainings, meetings, and professional development opportunities.\r\nQualificationsRequiredBachelor's Degree in Human Services or a related field.\r\nValid driver's license with reliable transportation.\r\nIntermediate computer skills, including proficiency with Microsoft Office applications.\r\nAbility to work effectively in both office and community-based settings.\r\nPreferredOne (1) year of case management experience.\r\nTwo (2) years of experience supporting individuals in:Behavioral health services\r\nAddiction recovery services\r\nCommunity-based human services programs\r\nSocial services or related fieldsKey CompetenciesCommunication SkillsAbility to communicate effectively and
