To learn more about our recent Series A funding, led by Andreessen Horowitz, please check out this article.As an Advanced Practice Clinician (APC) at knownwell, you will work alongside a team of committed physicians, APC, registered dietitians, registered nurses, and registered pharmacists, to provide comprehensive medical care, with a specific focus on the diagnosis, treatment, and prevention of obesity and obesity related conditions as appropriate in a primary care setting. This position offers an opportunity to make a significant impact on the lives of patients by helping them improve their overall health.Schedule: Monday- Friday with flexible scheduling, 32 patient contact hours minimum. Hybrid model of care. No scheduled evening/weekend responsibilities outside of phone On-CallEMR: AthenaOn-Call: Phone call only, in a rotation every 7-8 weeks for one (1) week at a time. Average call volume - <1 call per WEEK (as of 8.1.24). 24/7/365 physician back up.Requirements:Master's degree (MSN) and certification by one of the Nurse Practitioner certifying bodies (ANCC or AANPCB)) as a family nurse practitioner, or Master's degree in Physician Assistant studies and certification by the Physician Assistant certifying body (NCCPA)Unrestricted DEA licensure and prescribe authority (per state practice regulation)At least 2 years of clinical experience in an ambulatory care setting, family or internal medicine; experience in chronic condition management and weight management preferred. If no formal training in weight management, willingness to participate in knownwell clinical education and training plan for weight management and/or pursue OMA APC certification. Share a panel with MD/DO + other APCsEmbodies the knownwell mission and values of patient inclusivity in how you deliver careCoordinates with the broader care team to ensure the patient has the support they needDocuments as necessary to ensure accurate billing, including review of remote patient monitoring dataCommunicates with patients in a timely manner through our portal for questions and concerns that have been pre-screened to necessitate a clinician responseSee adolescents ages 12+ for metabolic health and primary careEnsure panel is open to any individual, regardless of their current health status and comorbiditiesListen to the aspirations of the patient and ensure care is tailored to the patientâs unique health journey.
