Primary Care Nurse Practitioner

Key Responsibilities: Conduct thorough physical examinations and assess patients' health status. Order and interpret diagnostic tests, including vital signs monitoring. Develop and implement individualized care plans based on patient needs and preferences. Provide family planning services, counseling, and education to patients. Utilize medical terminology effectively in documentation and communication with the healthcare team. Ensure adherence to established standards of care and practice guidelines. Perform clinical procedures while employing aseptic techniques to ensure patient safety. Work with individuals with developmental disabilities, promoting their overall health and well-being. Collaborate with healthcare providers to coordinate care, share insights, and develop treatment plans. Educate patients on disease prevention, wellness practices, and self-management strategies. Required Skills & Qualifications: Advanced clinical knowledge in primary care settings, with proficiency in assessing and managing common health conditions. Strong knowledge of medical terminology and documentation practices relevant to primary care. Ability to perform key tasks such as accurately taking vital signs, interpreting lab results, and developing treatment plans. Experience working with patients who have developmental disabilities is highly desirable. Excellent communication skills to engage patients in discussions about health education, lifestyle changes, and preventive care. Strong organizational skills to manage multiple patient cases efficiently and effectively. Ability to collaborate with multidisciplinary teams to deliver comprehensive patient care.
