This position offers an engaging opportunity for professionals committed to improving healthcare delivery through meticulous planning and collaboration.\nResponsibilities\nCoordinate patient discharges by collaborating with multidisciplinary teams including nursing, social work, case management, and physicians to develop comprehensive transition plans.\nFacilitate communication between hospitals, outpatient providers, hospice care agencies, and community resources to ensure smooth patient transfers.\nConduct thorough intake assessments and evaluate patient needs for appropriate placement or follow-up care.\nManage utilization review and utilization management activities to ensure appropriate level of care and resource allocation.\nOversee documentation within electronic health record (EHR) systems while maintaining strict adherence to HIPAA regulations.\nAssist in discharge planning by reviewing medical records, coding (including CPT, ICD-9, ICD-10), and verifying insurance authorizations.\nSupport case management efforts by identifying barriers to discharge and implementing strategies to overcome them.\nParticipate in multidisciplinary team meetings to review patient progress and coordinate ongoing care plans.\nMonitor compliance with hospital policies related to behavioral health, hospice care, pediatrics, trauma centers (Level I & II), ICU, PICU, OR settings, and emergency medicine.\nContribute to quality improvement initiatives focused on patient service excellence and efficient hospital operations.\nRequirements\nProven experience in hospital settings with a strong background in acute care environments such as ICU, PICU, trauma centers, or emergency departments.\nCritical care experience is highly preferred; familiarity with Level I & II trauma center protocols is advantageous.\nKnowledge of medical terminology, CPT coding, ICD coding (ICD-9 & ICD-10), and discharge planning procedures.\nExperience working within EMR/EHR systems with a solid understanding of documentation standards.\nStrong understanding of HIPAA regulations and confidentiality protocols related to patient information.\nBackground in social work or behavioral health services is desirable.\nExcellent organizational skills with the ability to manage multiple priorities efficiently.\nEffective communication skills for collaboration across diverse healthcare teams.\nFamiliarity with managed care processes and utilization review practices.\nAbility to adapt quickly within dynamic hospital environments such as hospital medicine units or specialty departments.\nIf you are passionate about facilitating smooth transitions of care through detailed coordination and collaborative teamwork, we encourage you to apply today.
