A company is looking for a Chronic Disease Coach (RN or Dietician). Key Responsibilities Assess member needs related to chronic conditions and coordinate personalized action plans Educate members and providers on chronic conditions and promote health program enrollment Maintain communication regarding member conditions and assist with referrals to support services Required Qualifications, Training, and Education Associate's degree in a job-related field or equivalent experience 3 years of professional health and wellness experience Working knowledge of word processing software Ability to work independently and prioritize effectively Must obtain health coaching certification within 4 years of hire
