Support transformation.Volunteers of America Delaware Valley is seeking a compassionate and clinically grounded Mental Health Case Manager to join our Behavioral Health & Reentry Services team. In this role, you will support individuals with mental health needs and justice-involved backgrounds through structured, person-centered care planning, risk assessment, and behavioral health intervention.You'll be part of a treatment team delivering specialized support that fosters recovery, accountability, and successful reintegration.What You'll Do:Complete clinical assessments and create individualized, client-driven service plansDeliver mental health interventions targeting risk/needs for special populationsFacilitate group programs, core curriculum workshops, and case management sessionsMonitor client progress and adjust care plans in collaboration with the treatment teamAssist clients with accessing housing, health care, and other community resourcesFacilitate house meetings and support reentry-related court and parole appearancesDocument all case activities and maintain records in accordance with funder guidelinesProvide drug and alcohol screenings and support overall program complianceParticipate in treatment team and training meetings; support peer staff functionsWhat You Need:Master's degree (MSW preferred); counseling or related human services degree consideredMinimum of 2 years of experience in mental health counseling with special needs populationsStrong communication, counseling, and group facilitation skillsUnderstanding of addictive behaviors and behavioral health treatment modelsCore curriculum facilitation certification (or willingness to obtain)Valid driver's license and ability to meet agency driving standardsAbility to work flexible hours, including some evenings and weekendsBasic computer proficiencyMust be able to pass criminal background checkBilingual fluency is a plusWhy This Role Matters:You will provide targeted mental health support in a structured, evidence-informed environment—helping individuals achieve stability and growth while navigating the challenges of reentry.
