Our residents enjoy worry-free living in a warm and friendly community with services to meet their individual needs.\n\nIs accountable for implementing the Dimensions Program, encouraging participation and ownership from all staff. Manages and supervises the programmatic, physical and educational operations of Dimensions Community in meeting the resident's/family's needs and expectations. Collaborate with the interdisciplinary team and provides education to staff/families to adequately equip them to interact effectively with residents experiencing dementia.\n\nResponsibilities\n\nProvides leadership and supervision to staff, volunteers, and/or students:\nSupports an effective staffing model and works with the Director of Health Services (DHS) and/or staffing designee to ensure efficient use of staffing resources.\nIn Partnership with the DHS, provide ongoing staff support.\nIn Partnership with the DHS, provide employee orientation and on-going education/staff development.\nProvides resident/employee feedback to the DHS.\nMaintains department records for resident attendance at activities.\nImplement Dimensions Program.\nEstablishes and supports an environment conducive to the provision of quality services and high-level customer satisfaction:\nCommunicate and maintain good working relationships with residents, families, staff, etc.\nProvide overall program direction and on-going development.\nDevelop monthly activity calendar for Dimensions neighborhood in collaboration with the Activities Director.\nResponds effectively to customer concerns.\nRoutinely reviews and follows-up on satisfaction surveys.\nEnsure proper programming supplies are available for residents, staff, etc.\nParticipates in Dimensions Program assessment.\nImplement and maintain the Dimensions Program:\nServes as the Dimensions Champion for the site, attending Ebenezer’s Dimensions Trainings and ongoing Dimensions Champions training.\nCoaches and mentors care team, other staff members and volunteers in best practices of the Dimensions Program to ensure effective approach and interaction with persons living with dementia. (e. not “living with memory loss”, as it’s not all about memory loss)\nCollaborates with nursing to maximize recommended contributions to resident care plan from current Cognitive Performance Tests (CPT), My Way, Advance Care Planning and Global Deterioration Scale (GDS)\nWorks with the team to utilize psychosocial (i.e. non-pharmacological, such as aromatherapy, music, massage, etc.) approaches and interventions.\nEnsures Dimensions environment is conducive to enhancing resident experience and encouraging engagement.\nFacilitates, coordinates, and supports Site’s Care Partner Support group.\nDepending upon site, may manage the journey program.\nDevelop program and activities reflective of Dimensions Community resident interests:\nAssesses Dimension resident’s areas of interest and needs. Designs and implements programs to meet identified psychosocial needs of the residents.\nEnsures, through observation and evaluation, that residents maintain their individualized highest level of performance and independence.\nPlans, coordinates, and supports day-to-day activities and programming in Dimensions Community.\nEnsure execution of programming through Activity Assistants, Resident Assistants and/or other available staff.\nEncourages resident participation in activity programming.\nCommunicates and coordinates programming with Activity Director and other departments.\nAssists the Activity Director in hiring and training Activities Assistants.\nOffers suggestions and innovative ideas to improve the Dimensions Community to the Executive Director.\nSupports a marketing plan aimed at full occupancy:\nReviews Dimensions Community occupancy level regularly.\nParticipates in conducting resident/family tours of community.\nDrives Ebenezer van/bus for resident outings:\nReviews and completes Vehicle Requirements – Transporting Residents Checklist prior to each resident outing.\nMaintains a log of transportation services and tasks performed throughout the driving period.\nResponsible for the safety and welfare of passengers.\nPractices sound, safe judgment when performing transportation services including but not limited to: fire/disaster procedures and infection control procedures.\nRecognizes and acts appropriately on safety hazards and incidents.\nFollows all laws with regard to operating a motor vehicle.\nImmediately reports any traffic violations or accidents.\n\nRequired Qualifications\n\n1 year One to Three Years\nMS Office\nHigh level communication skills and the ability to interact effectively with older adults\nDrivers License Current Driver’s License (in good standing)\nMust be insurable under the employer’s/communities Auto Insurance Coverage\nMedical Exam Certification\nMust be at least 18-years old\nMust complete and pass in-house transportation curriculum course and pass driving evaluation. within 30 Days\n\nPreferred Qualifications\n\nA.A./A.S.
