Airport Blvd, Sanford, FL 32773Remote work will not be a consideration. This is a highly independent and responsible Advanced Practice Registered Nurse position with the HIV/Specialty Care Clinic. This position is part of a multidisciplinary team that ensures proper care and treatment to clients who are seen for the following services: HIV/AIDS, PrEP, nPEP, and Community Outreach.Your specific responsibilities include:Providing services for persons living with HIV/AIDSParticipating in enhanced care coordination services to persons living with HIV in Seminole CountyIncreasing the numbers of persons living with HIV/AIDS who engage and remain engaged in HIV primary-specialty careIncreasing the number of persons maintaining viral load suppressionWorking independently gathering data, preparing reports, and providing day-to-day operational assistance to the Program Manager, and other staff as neededApplying advanced nursing principles and techniques in the care and treatment of clientsPrescribing, administering, and documenting medications ensuring the Rights of Medication AdministrationProviding holistic care that may include diagnosing and treating sexually transmitted infections, hepatitis, tuberculosis, conducting initial/annual physicals, well woman health exams (including pap smear, pelvic exam, & breast exam when warranted), and counselingPossessing the ability to provide comprehensive primary care across diverse patient populationsConducting thorough assessments, diagnosing and managing acute and chronic conditions, and providing evidence-based treatment that prioritizes patient education, prevention, and wellnessMaking appropriate referrals as indicated, evaluating immunization status, and administering immunizations per protocol using correct techniquesLogging administered vaccines into Florida Shots, Health Management System (HMS) and Care Ware Provide Enterprise SystemEntering medication refill requests into HMS EmdeonAccurately documenting all services providedReviewing lab results and providing follow-up to those assigned as abnormal or priority especially viral loads and CD4 countsAssessing client's well-being including medication adherence, reasons for missed appointments, housing conditions, transportation, providing and implementing clinical recommendationsProviding care coordination especially to newly diagnosed people living with HIV/AIDSServing as liaison with case management team, specialty care providers/partners and State Telehealth ProvidersReviewing and authorizing prescription requests made by pharmaceutical providers and clients according to treatment needed by each clientProviding education to clients, partners, team members and communityAssisting Program Manager and/or designee with the coordination of pharmacy services for approved clients as well as with additional areas including pilot studies, surveys, focus groups, reports, etc.Maintaining 100% compliance with confidentiality protocols as well as security and integrity as determined by the Florida Department of Health HIV/AIDS Section Bureau of Communicable Diseases, as well as all other databases, including Medicaid (FLMMIS), Health Management System (HMS), Availity, Change Revenue (Optum), STARS, Florida Shots as assignedReporting immediately to the Manager or designee of any problems/complaints clients report, as the needs of the client's mandateEntering all client encounters both face to face, telehealth, and by phone into the Health Management System (HMS) and Provide Enterprise System for tracking and reporting purposesSubmitting required reports, maintaining assigned records, completing/returning all paperwork pertaining to job duties accurately, legibly and in proper orderAnswering incoming telephone calls in a timely manner; speaking clearly and concisely, exercising tact and providing accurate information; accurately and efficiently providing support in other clerical areas as needed; interacting positively and professionally with nurses and other clinic staff to assure client needs are metParticipating in HIV treatment adherence, HIV/AIDS education/prevention and care management trainings and webinarsProviding information and training as needed in the field to DOH-Seminole Specialty Care and clinical division staff, community-based organizations, and case managersRemaining current with changes and usage of HRSA policies and proceduresAssisting with chart reviews and other QA/QI activities to improve clinic functions and to meet DOH-Seminole's mission statement and goalsParticipating of the Quality Advisory Committee, maintaining appropriate relations with internal external partnersActively participating in case discussions or case conferences for the clients to improve assessment and to sustain retention in careParticipating in team meetings and all meetings convened by the ADAP Program, DOH-Seminole, Community PartnersMaintaining appropriate relationships with Key Point of Entry (KPOE) into the healthcare systemTranscribing minutes from meetings and producing minutes for distribution within 2 daysConducting outreach activities for potential clients to educate and promote the availability of servicesIncumbent will participate in health fairs, community events and collaborate with providers among other activitiesThis position may be required to work before, during and/or beyond normal work hours or days in the event of an emergencyEmergency duty required includes working in special needs or Red Cross shelters, or performing other emergency duties including, but not limited to, responses to or threats involving any disasters or threat of disaster, man-made or naturalThis position will maintain confidentiality and security of records as specified by departmental regulations and lawsRequired Knowledge, Skills, and Abilities:Bilingual preferred (English/Spanish preferred)Knowledge of HIV/AIDS and STD, terminology, principles and analytical techniques of interviewing; data collection and analysis principles and proceduresCustomer service skillsAbility to prepare reports, understand and apply applicable rules, regulations, policies and procedures relating to Ryan White, ADAB State and Local Specialty Care HIV/AIDS Programs, understand and follow legislative policy direction as established in Federal and Florida Statutes; deal with the public in a tactful and courteous manner; work independently, plan, organize and coordinate work assignments, communicate effectively, establish and maintain effective working relationships with others;
