Physical Therapist (Full-Time)
Job Summary
Physical Therapist (“PT”) to provide quality care to our patients. The PT evaluates and treats patients, communicates, and provides education to patients/their families, and maintains all necessary documentation of services in the patient’s medical records. Initiates discharge planning for physical therapy. The PT implements the treatment plan through direct treatment of the patient, delegating a portion of the treatment plan to appropriate supportive personnel, and coordinates the treatment program with other health care personnel as appropriate. The PT will also analyze rehabilitation goals and collaborate with the physician and rehab team in order to coordinate the most effective physical therapy program.
Responsibilities and Duties
Essential Responsibilities and Expectations
Evaluate patients promptly, and per facility policies and expectations, upon receiving physician referral for treatment
Develop effective treatment plans
Obtain approval for services from referring physician
Treat patients according to treatment plan approved by physician
Communicate regularly with supervisor, other members of the rehabilitation team, and family regarding patient treatment plan, progress, prognosis, and planned discharge
Record evaluations, daily treatment notes, re-assessments, and discharge summaries per Company policy and procedure
Complete all required documentation from your daily treatments/interventions prior to leaving the facility
Educate patient families and caregivers in treatment and any applicable maintenance programs
Participate in discharge planning
Secure necessary durable medical equipment (e.g., orthoses, walkers, canes, standers, wheelchairs, etc.) for patients to facilitate independence
Submit all billing and necessary information prior to leaving the facility
Supervise Physical Therapist Licensed Applicant (PTLA), supervise licensed Physical Therapy Assistants (“PTA”), and supervise a Rehabilitation Aide (“RA”) appropriately
Report any problem areas/equipment within each facility to the appropriate supervisor immediately
Comply with State Licensing Board of California, Title 22, CMS, and California Labor Board guidelines
Comply with Company rules, policies and procedures
Comply with all applicable safety rules, policies, and procedures
Report to work regularly and on-time
Additional Responsibilities and Expectations
Be willing and able to act as a mentor for new or recently hired PT/PTA
Provide necessary training and guidance to student interns as assigned, including:
Supervise students regularly
Providing clearly defined, career-related (i.e., non-clerical) responsibilities and tasks in accordance with the students’ academic programs
Informing student interns about and ensuring their compliance with Company, facility, state, and federal guidelines
Completing any paperwork required by the Company or the students’ academic program

Assumes other duties as assigned.

Ability to transfer or assist in transfer of up to 100 lbs.
Ability to stand and/or work 80% of an 8-hour shift.
Ability to bend, reach, squat, move as required by patient treatment plans.

Visual and hearing acuity to perform job-related functions.

Education, Skills, and Credential Requirements
To perform the PT job successfully, you must be able to perform each essential responsibility satisfactorily. Required
Licensed as a PT in the State of California.
No current citation or probation listed on the Physical Therapy Board of California.
Pass all Background Checks (e.g., Criminal Offense, Sex offender Registry, etc.)
A physical examination from a licensed physician indicating to be able to perform job duties and
free of communicable disease.
A negative TB Test taken within 12 months as a current employee or taken within 3 months as a new hire or a rehire, OR a clear Chest X-ray taken within 5 years and a TB Questionnaire that must be completed each rolling year.
A valid CPR certification.

Workplace Environment
The PT will primarily be working in our outpatient clinic. This is an active patient care environment. This environment has no hazardous Health and Safety exposures. However, Health and Safety precautions must be taken at all times.
Salary Range: $42.00 to $60.00
