We are committed to providing compassionate, evidence-based care to individuals experiencing substance use disorders, mental health challenges, and homelessness.We are currently hiring a full-time Stratford Case Manager to join our multidisciplinary housing and behavioral health team in Salt Lake City.About The RoleThe Stratford Case Manager provides comprehensive case management and supportive services to residents living in First Step House permanent supportive housing program. This position works closely with individuals experiencing serious mental illness, substance use disorders, and co-occurring conditions to support housing stability, recovery, and community integration.This Role InvolvesBuilding supportive relationships with residentsAssisting clients in identifying goals and service needsCoordinating care and community resourcesSupporting engagement in treatment and recovery servicesFacilitating life skills and supportive groupsProviding transportation and appointment coordinationCollaborating with multidisciplinary clinical and housing teamsServices are provided both onsite and in residents' homes using trauma-informed, recovery-oriented, and strengths-based approaches.This is a meaningful opportunity to make a direct impact by helping individuals build stability, independence, and long-term recovery.Salary$46,353.60 annuallyQualificationsA Bachelor's degree and an active license in the State of Utah as a Social Service Worker (SSW) is strongly preferred.
