We invite Advanced Practice Providers to discover all that we can do when we bring healing hearts, inquisitive minds, and progressive visionaries together within our Hospital Medicine team at Hugh Chatham Health.Position Highlights100% inpatient adult medicineHospital-based practice with collaborative physician and Advanced Practice Provider team modelAdult and geriatric patient population7 days on / 7 days off schedule, or as otherwise specified by site needsProcedures not requiredCollaborative interdisciplinary support including physicians, nursing staff, case management, consulting services, and administrative support staffOpportunity to participate in education and mentorship with APP students and APP fellowsCentralized professional support through the Center for Advanced PracticeEligible for annual performance-based bonus opportunities tied to physician/APP team goalsCME allowance providedOpportunity to join and support a growing Hospital Medicine program within a newly integrated Advocate Health partner locationPosition ResponsibilitiesPerform comprehensive history and physical examinations for hospitalized adult patientsEvaluate, diagnose, and manage acute and chronic medical conditions in the inpatient settingDevelop and implement patient-centered treatment plans in collaboration with supervising physiciansOrder, interpret, and monitor laboratory studies, imaging, and diagnostic testingMonitor patient progress and respond appropriately to changes in clinical conditionPrescribe and adjust medications in accordance with North Carolina regulations and DEA guidelinesCollaborate closely with hospitalists, nursing staff, consulting physicians, care management, and ancillary teams to coordinate careParticipate in interdisciplinary rounds and discharge planningFacilitate communication with primary care providers and patient families regarding ongoing care needsMaintain accurate and timely electronic medical documentationEducate patients and families regarding diagnoses, treatment plans, medications, and follow-up careParticipate in quality improvement initiatives and patient safety effortsMaintain compliance with Joint Commission, HIPAA, OSHA, and organizational standardsCandidate QualificationsGraduation from an accredited Nurse Practitioner or Physician Assistant program requiredCurrent North Carolina license or eligibility requiredCurrent prescriptive authority and DEA licensure requiredBLS and ACLS certifications requiredAcute Care certification required for Nurse Practitioner candidatesMinimum of 1–2 years of inpatient, hospital medicine, internal medicine, emergency medicine, critical care, or related acute care experience preferredStrong clinical judgment, critical thinking, and interpersonal communication skills requiredAbility to work independently in a fast-paced and collaborative hospital environmentNurse Practitioner CandidatesMaster’s Degree or Doctor of Nursing Practice requiredAcute Care certification requiredCurrent RN and APRN licensure requiredCertification from a nationally recognized certifying body requiredACNP, AGACNP, AGNP, PNP, or PNP-AC certification preferred depending on population servedPhysician Assistant CandidatesGraduate from an accredited Physician Assistant program requiredMaster’s Degree preferredNCCPA certification preferredAbout Hugh Chatham HealthHugh Chatham Health is a community-focused healthcare organization dedicated to delivering high-quality, compassionate care to the communities of northwest North Carolina.
