About the RoleTheCommunity First Choice (CFC) Care Managerplays a vital role in supporting individuals who need assistance to live safely and independently in their homes and communities. This position is responsible for developing, implementing, and monitoringindividualized, person-centered care plansthat follow CFC program guidelines and promote dignity, autonomy, and successful community living. The Care Manager collaborates with clients, families, healthcare providers, and community partners to coordinate access to essential services. Through ongoing assessment and monitoring, the Care Manager ensures that care plans remain responsive as client needs change. This position directly enhances quality of life for older adults and individuals with disabilities by fostering independence and community integration. Minimum QualificationsBachelor’s degree in Social Work, Nursing, Psychology, or a related human services fieldAt leasttwo years of experiencein care management, case management, or a similar role in social services or healthcareKnowledge ofCommunity First Choice (CFC)program guidelines and related state/federal regulationsStrong understanding of community-based resources and social service systemsValid driver’s license and reliable transportation for home and community visitsKey ResponsibilitiesConduct comprehensive assessments of clients’ physical, behavioral, emotional, and social needs to inform individualized care plansDevelop, coordinate, and monitor in-home and community-based services in alignment with CFC program standardsCollaborate with healthcare providers, community agencies, and family members to ensure seamless service deliveryMaintain accurate, timely documentation of assessments, care plans, and client progress in accordance with regulatory requirementsEducate clients and families about program rights, available resources, and self-advocacyIdentify gaps in services and advocate for clients to receive appropriate supportsParticipate in interdisciplinary team meetings to evaluate client progress and adjust care goals as neededEnsure compliance with agency policies, program guidelines, and professional ethical standardsSkills & CompetenciesSuccessful Care Managers demonstrate: Strong interpersonal and communication skillsCritical thinking and problem-solving abilitiesExcellent organization and time management, with the ability to manage multiple casesThorough knowledge of community resources and social service systemsAdvocacy skills and commitment to person-centered careProficiency with EHRs, databases, and digital toolsCultural humility and the ability to work effectively with diverse populationsBenefitsAOASCC offers a comprehensive and competitive benefits package designed to support employee well-being, work-life balance, and professional growth.
