Administers or supports diagnostics testing to support diagnosis and treatment plans and participates in a community effort to support chronic disease management. May include evaluating patient responses to prescribed treatment plans, testing, and procedures and developing ongoing plans./pulliDevelops exercise prescription and risk factor modification by incorporating comorbidities and musculoskeletal disorders, per established program (ACCVPR, ACSM, ATS, ACC, NASM) standards and guidelines, as evidenced by random chart review./liliDevelops Individual Treatment Plans through physical assessment and testing to include exercise and strength training programs, patient education, and behavioral health support specific to each patient to achieve optimal outcomes./liliAdministers or supports diagnostic testing for patients./liliDemonstrates proper instruction and technique for performing appropriate exercise and strength training as needed to ensure patient safety and adherence to exercise prescription, as evidenced by direct observation./liliImmediately reports significant changes in patient clinical status to appropriate members of the healthcare team and assists with emergency care according to department policy, as evidenced by direct observation./liliProvides validation of clinical decisions for other team members./liliSupports and engages in clinical outcomes and quality improvement initiatives./liliUtilizes standards, guidelines, and pathways for care delivery. Determines process for delivery of care from direct patient and family communication, technology, and other healthcare team member collaboration./liliMay perform additional duties as assigned./li/ulpMinimum Qualifications:/pulliEducation: Bachelors Degree Exercise Physiology, Exercise Science, or Kinesiology/liliExperience: Relevant clinical experience in a hospital, clinical setting or clinical internship completed through education coursework/liliCertifications: BLS and ACLS from the American Heart Association required within 6 months of hire; American College of Sports Medicine - Certified Exercise Physiologist (ACSM CEP) for Cardiac and Pulmonary Rehab upon start OR American Association of Cardiovascular and Pulmonary Rehabilitation - Certified Cardiac Rehabilitation Professional (AACVPR CCRP) for Cardiac and Pulmonary Rehab upon start/li/ul/div
