In this role, you will oversee our Transitions Program, which assists individuals experiencing homelessness with learning independent living skills to create and maintain stable independent living. You will also oversee our Sage House Program, which provides free housing for individuals enrolled in our Transitions Program. Nurstead’s Transitions and Sage House Programs are currently spread across three counties in Eastern New Mexico and have successfully helped dozens of people transition from homelessness to stable living situations over the past two years. Find out more about our Sage House and Transitions Programs at: https://nursteadmh.com/sage-house/\nResponsibilities\nSupervise and mentor a team of community support workers, peer support workers, and case managers to deliver high-quality support services.\nImplement case management strategies tailored to individuals with mental health conditions or addiction challenges, employing motivational interviewing techniques to promote positive outcomes.\nDevelop partnerships with community organizations to expand access to housing resources and support services for underserved populations.\nTo provide individualized, ongoing guidance, coaching, and support.\nTo provide training in the use of personal and community resources.\nAssist in developing formal and informal community supports.\nAssist the person served in increasing social support networks of relatives, friends, and/or significant others.\nTo offer encouragement in times of crisis.\nTo advocate on behalf of persons with behavioral health problems to protect the client’s rights and to assist in reducing associated stigma.\nTo work in cooperation with other providers, family members, or significant others involved in the client’s recovery plan.\nTo attend Agency staff meetings, Case Conferences, and Individual and Group Supervision.\nTo observe all rules of confidentiality relating to clinical information and treatment, both internally and when dealing with external agencies and/or individuals.\nTo participate in staff training and development.\nTo maintain professional standards at all times.
