Job Description:Primary Care Opportunity- Located in Fairfax, SC - Charleston 85m - Full-Time, permanent positionQualifications:MD or DO from a 4-year accredited medical schoolBoard certified or board eligible (in process of certification)Active South Carolina medical license with prescriptive authorityDEA certification with appropriate scheduled drug approvalsBLS required; ACLS preferredKey Responsibilities:Conduct patient assessments and create comprehensive management plansOrder and interpret labs, imaging, and other diagnostic studiesDocument clinical encounters in the EHR, including after-visit summariesParticipate in care coordination, referrals, and team-based workflow processesEducate patients on self-management and treatment adherenceAddress and document barriers to achieving health goalsParticipate in pre-visit huddles and ongoing clinical collaborationProvide emergency medical care and oversee medication dispensing when neededSupervise mid-level providers and consult with other clinicians as necessaryEngage in community outreach including health fairs, immunization events, home visits, and sports physicalsMaintain active membership in local, state, and national medical societiesAttend staff/provider meetings, continuing education events, and case presentationsParticipate in quarterly chart reviews and Health Care Collaborative effortsPerform other duties as assigned by supervisorPractice Approach:Team-based care focused on patient preferences and outcomesEmphasis on care coordination and population health managementUtilization of EHR for documentation and communication
