South Carolina Licensed Population Health Manager

A company is looking for a Population Health and Manager of Post Acute Care Program Lead. Key Responsibilities Develops, implements, evaluates, and maintains health management programs Coordinates care transitions for members from acute hospital care to post-acute settings Monitors utilization and evaluates member outcomes to ensure cost-effective care Required Qualifications Bachelor's degree in a job-related field 5 years of clinical experience or experience in developing population health programs, with 2 years in a supervisory role Active, unrestricted RN licensure or equivalent licensure for Social Worker or Counselor Strong analytical and data interpretation skills Working knowledge of Microsoft Office software
