*Overview* The Director of Shared Living (DSL) will be required to make monthly home visits to all Clients in Shared Living. This is to ensure that all Shared Living homes are in compliance and to address any issues with the Client and/ or Shared LIving Contractor(S) with Shared Living Services. The DSL will also ensure that site files in the homes are complete (or items are requested) with all required paperwork. Other duties may also be assigned as determined. QUALIFICATIONS: High School Diploma or equivalent. 2 + years experience and/ or education with IDD population. Other Requirements: 1) Maintain a current valid driver's license of the state of residence and be determined as an "insurable driver" by the automobile insurance carrier for CLASS unless other arrangements can be made in advance. 2) Comply with all federal, state and local laws regarding comminicable and infectious diseases and CLASS LTD policies rules and practices regarding medical examination or immunizations. 3) No records of convictions, administrative findings or history of abuse, neglect or exploitation of children or adults unable to protect their own interest, and conviction of other crimes, including, but not limited to crimes against persons, sex crimes, crimes against property, violation of personal rights, public safety, public morals, or any crimes that would be classified in Kansasas a felony or deemed unacceptable for employment with CLASS. 4) Ability to communicate effectively with others 5) Read and comprehend written materials generated by CLASS, e.g. Policies and Procedures. 6) Compose written material as required. 7) Ability to travel across the 4 counties that CLASS serves (Cherokee, Crawford, Labette, and Montgomery). 8) Works and honors commitments to participate as an effective member on teams. e.g. Specialty Teams, PCSP teams etc. 9) Evening and weekend availability may be required. ESSENTIAL FUNCTIONS: SHARED LIVING RESPONSIBILITES: 1) Recruitment of Shared Living Homes. Markets & solicits SL homes; promotes SL clients. 2) Conducts home visits prior to placement(s) and assures KDADS & CLASS, health and safety expectations are being met. 3) Assists with identification, monitoring and coordinating the training needs of Shared Living Contractors (SLCs). 4) Maintains a database of current SL homes and potential home providers. 5) Provides paperwork training and assists new SLCs in correctly completing required paperwork. 6)Maintains close working relationships with KDADS, CDDO and CLASS staff members. 7) Reviews CLASS-SLC contracts annually and SLC-Client agreements/acknowledgement as changes occur. 8) Visits SL Homes at least MONTHLY to ensure quality services are being delivered and documents any corrective action plans when needed. 9) Monitors and follows-up on any corrective action plans and documents outcome/status. 10) Communicates and works with necessary team members relating to SL Services, including attending BASIS and PCSP meetings and assisting in the development of the PCSP and MCO Service Plans when necessary. 11) Other duties as assigned. EDUCATIONAL, TRAINING AND SELF-DEVELOPMENT- PRINCIPAL ACCOUNTABILITIES 1) Completes required training/certification, continuing professional development, and keeps informed of relevant resources and information. 2) Enhances community and agency awareness of issues affecting clients by assisting in providing counseling/training to client and families, participating in staff/team meetings, and sharing knowledge/training with co-workers. 3) Markets and promotes SL services through information sharing and education, while reporting to the CEO/President or the VP for Services. 4) Other duties as assigned.
