Job Summary: The Registered Nurse Case Manager will provide skilled nursing care, case management, and hospice care to individuals facing advancing age, frailty, and serious illness. Responsibilities include patient assessments, medication administration, treatment planning, and collaborating with an interdisciplinary team.
