Registered Nurse Case Manager
Job Summary: The Registered Nurse Case Manager will provide skilled nursing care, case management, and hospice care to individuals facing advancing age, frailty, and serious illness. Responsibilities include patient assessments, medication administration, treatment planning, and collaborating with an interdisciplinary team. Strong communication and collaboration skills are essential for interacting with patients, providers, and team members.Location:Eureka, California, United StatesResponsibilities:- Provide skilled nursing care.- Perform patient assessments.- Administer medications.- Develop and implement treatment plans.- Collaborate with the interdisciplinary team.- Float between like units based on need.Required Skills & Certifications:- Active Registered Nurse license in the state of working.- Clinical Assessment, Medication Administration, and Treatment Planning skills.- Minimum of 2 years of acute care experience.- Interdisciplinary Team Collaboration skills.- Excellent communication and interpersonal skills.- Ability to work effectively in a fast-paced environment.- Bachelor's degree in Nursing or related field.- Driver's license, car insurance, and automobile for field work.Preferred Skills & Certifications:- Case Management experience.Special Considerations:- Required to work every other weekend.- Must include all requested time off (RTO) at the time of submittal.Scheduling:
