This position collaborates with the Clinical Program Coordinator and Manager in program design, implementation and evaluation.Essential Functions and ResponsibilitiesProactively contributes to Eskenazi's mission: Advocate, Care, Teach and Serve with special emphasis on the vulnerable population of Marion County; models Eskenazi values of Professionalism, Respect, Innovation, Development and ExcellenceServes as a role model and clinical resource for other Care Coordinators and Support SpecialistsAssists Care Coordinators and Support Specialists in determining appropriate interventions to aid clients in reaching their treatment goalsOffers input for staff performance appraisalsMonitors and assists staff in maintaining productivity standardsCollaborates with Clinical Program Coordinator and Manager in program design, implementation and evaluationIn collaboration with the MD/CNS and treatment team, formulates, develops and revises plan of careInsures the timely completion of all client documentation per policyUses discretion and independent judgment to develop treatment plans, in consultation with the treatment teamPrioritizes and subsequently coordinates treatment activities and both internal and external referrals for a caseloadInterviews patients and significant others to gather information for Biopsychosocial assessments and reassessments and ANSA/CANS assessmentsAssesses patient treatment progress to evaluate success of treatment planning, and updates treatment plans at intervals indicated by guidelinesAdvocates for clients to ensure access to needed servicesImplements treatment plans, coordinates service revisions based on treatment plansMonitors client progress, reporting to the team successes and challengesIdentifies and establishes a set of alternative resources in the community to be accessed to meet a wide variety of patient needs related to all life domainsIdentifies any high risk patient symptoms or circumstances that would result in risk for self-harm or harm to others, and notifies appropriate higher clinical level staff for consultationDocuments client treatment activities and client progress toward treatment goalsCompletes appropriate legal documentation as indicated on treatment planInterprets and applies service and billing procedures according to client needs; completes billing according to guidelines for timeliness and accuracyWorks with family, significant others and community supports to assist client in meeting goalsAssists clients in accessing community resources; keeps accurate and updated information about resourcesWithin scope of practice, assists other team members in covering client needsAlters activities/behaviors to reflect and ensure adequate care appropriate to the age of clients servedIn Residential Services:Coordinates and supervises facility staff in the development, implementation, and integration of activities in 16-24 hour residential facilities to ensure the safety of the clients and support a smooth operation of the facilityPerforms crisis intervention on-site as well as an on-call basisEnsures the facility environment meets all required safety and cleanliness standardsEnsures adequate staff coverage in the facility and may be required to work alternate shifts/days to support thisEnsures that processes are in place to provide a tracking system so that all client funds are allocated properly and recordedJob RequirementsBachelor's degree in social work, substance abuse, education, counseling, psychology, sociology, criminal justice or related field, plus one year of related work experienceMay substitute a Master's degree in social work, substance abuse, education, counseling, psychology, sociology, criminal justice or related field if no prior experience in mental healthValid Indiana driver's licenseKnowledge, Skills & AbilitiesAbility to fulfill duties noted in areas' specific competencies (Adult, Acute, Children's or Addiction Services)Use of discretion and independent decision making regarding client safety and careStrong interpersonal skills; ability to be both clear and concise in written and verbal communicationWillingness and ability to carry out directives from other team members; ability to teach and mentor othersDemonstrated leadership and team building abilitiesAbility to use communication skills appropriate to the age of the clientAbility to interpret and apply complex directives/information/regulationsAbility to use crisis intervention skillsAbility to implement community and strengths based treatment interventionsKnowledge of psychiatric and addiction diagnoses and understanding of their impact on behavior and all life domainsKnowledge of psychotropic medications, their uses and side effectsSandra Eskenazi Mental Health Center's primary mission is to serve individuals with serious mental illness and chronic addiction as well as seriously emotionally disturbed children and their families.
