PCA HHA-12 Hour Shift Night/Day

Constant Care Health Services is a Licensed Home Care Agency that provides Excellent Services to our Community. We are currently seeking 12 Hour Night and Day HHAs. HealthCare Heros to join our Constantly Caring team. QUALIFICATIONS: Current Home Health Aide Certificate in the State of New York RESPONSIBILITIES: Assist with patients Personal Care needs: personal hygiene, bathing, dressing/undressing, and ambulation together with any household task (eg making beds, changing linen, laundry, light housekeeping, etc) essential to the patients’ health in accordance with the patient's individualized Plan of Care. Plan/Prepared nutritious meals and assist in feeding patients as directed. Document services provided in accurate and timely manner in accordance with company policy and personal care plan. Maintain Confidentiality at all times Report any changes in the patient’s condition or incidents to RN Supervisor Carry out other duties Accompany patients to their doctor visits
