We are proud to be opening our newest treatment center in Agawam, MA, focused on Substance Use Disorder (SUD) treatment for adults.Join us as we build a new program dedicated to recovery and make an immediate impact on the lives of clients and their families.Position SummaryWe are seeking a dedicated and organized Case Manager to support clients and families throughout the continuum of care. This role is essential to client success and includes coordinating services, communicating with referral sources and external providers, assisting with admissions processes, and supporting aftercare planning to ensure smooth transitions following treatment.Key ResponsibilitiesCoordinate client care activities, including appointments, schedules, and treatment planningCommunicate with referral sources and outside providers to ensure continuity of careSupport the admissions process by verifying documentation, scanning records, and initiating communication with external providers as neededConduct orientations with clients and families in collaboration with the primary therapistProvide case management services including transportation coordination, case management sessions, and support related to legal or employment needsFacilitate educational or skill-building groups to support clinical programmingDevelop and document comprehensive aftercare plans in collaboration with the treatment team and business development staffProvide aftercare documentation to appropriate external providers and ensure follow-up coordinationMaintain accurate and timely documentation in the electronic clinical record system (CareLogic)Participate in treatment team meetings and maintain collaborative communication with counselors and staffEnsure compliance with confidentiality standards, licensing requirements, and program policiesComplete required trainings and perform additional duties as assignedQualificationsEducationHigh School Diploma or GED required; bachelor’s degree in a related field preferredTwo years of previous case management experience preferredExperienceMinimum of 2 years of case management experience preferred, ideally in behavioral health or substance use treatmentSkillsStrong organizational, communication, and interpersonal skillsAbility to manage multiple tasks and meet deadlinesKnowledge of community resources and referral processesRequirementsAbility to maintain professionalism, confidentiality, and accurate documentationWhy Join Pyramid Healthcare?Opportunity to be a founding team member at a newly launched treatment programMake a meaningful impact helping clients and families navigate recoveryCompetitive pay and opportunities for advancementSupportive, mission-driven team environmentOngoing training and professional developmentInterested in joining a team dedicated to recovery and compassionate care?
