As a team member at DuPage County, you will also have access to time off with pay on Designated Holidays or holiday pay if required to work, Vacation time, Sick time and Personal days.\nTo learn more about the cost and coverage of the benefits plan, visit Employees Benefit Information\nFor detailed information on paid time off, visit: AFSCME Council 31 - Care Center.pdf\n\nThis position is assigned to the AFSCME (Care Center) Bargaining Unit.\n\nDuPage County is an Equal Opportunity Employer EXAMPLE OF DUTIES Responsibilities Include:\n\nCoordinates with other departments to collect and input the information required for the federally mandated assessments (MDS)\nMonitors input to ensure accuracy, timeliness and compliance with reimbursement authorities and standards\nReviews clinical charts and condition of patients\nPlans and coordinates resident's plan of care with other members of the interdisciplinary team\nDiscusses resident condition with physicians, nursing staff and family members\nCompletes the computer database required for the MDS\nOversees documentation to comply with Medicare or Illinois Department of Public Aid reimbursement rules\nLeads care plan conferences and other interdisciplinary meetings\nInterprets resident charts to accurately complete sections of the MDS\nReviews resident charts for contradictions in information and researches any contradictions to determine what is valid\nStays current with job related professional developments, regulations and standards\nRecommends hires and promotions, directs and evaluates employment decisions for all assigned positions\nMay perform the duties of a Registered Nurse as assigned\n\nSUPPLEMENTAL INFORMATION\nPHYSICAL REQUIREMENTS\nThe physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of the job.
