Title: Social Worker Job Type: Full-time Salary: DOE & competency, will be discussed after interview. Heath care center in Monrovia Hiring full time position Social Worker, bilingual in English / Chinese (Mandarin and Cantonese is strongly preferred!). Master degree in either Social Work, Sociology, Psychology, Gerontology or related field, or Bachelor degree of social work or in related field, plus 2 or more years of social work experience in health care center setting. Must be able to legally work in the U.S. Must be able to speak Mandarin. Responsibility: /Monitor and assess participant’s physical/mental/emotional status, provide encouragement and support accordingly. /psycho-social assessments for center participant. /Provide individual support and consultation to emotionally/mentally distressed individuals. /Process and document inquiries in a timely fashion. /Meet with family member, determine appropriate programs for resources referral. /Maintain professional stance under critical situation; be sensitive and caring to the aging / vulnerable population. /Provides case coordination include English to Chinese translation/interpretation of governmental, health care, and community programs and policies. /Complete quarterly assessment, progress notes, six-month reassessments, and initial assessment. /Discharge planning for enrolled members. /Develop monthly social support group for seniors. /Serve as a member of the multi-disciplinary team and participate in initial assessments, care planning, and on-going re-evaluation of participant's needs. /Be sensitive to environmental safety.
