Job description What You'll Do - Your Purpose in Action • Champion resident wellness by identifying and responding to evolving care needs, ensuring residents receive the highest quality of personalized service. • Conduct timely assessments at move-in, 30-day, semi-annual, and status-change intervals in partnership with the Health & Wellness Director (HWD). • Lead monthly wellness visits and monitor clinical status, vitals, weights, and medication profiles with care and accuracy. • Respond to incidents such as falls or changes in condition with proper evaluation, documentation, and follow-up. • Maintain accurate wellness documentation and ensure compliance with Thrive policies and regulatory standards. • Communicate effectively with residents, families, and healthcare providers to ensure collaborative care planning. • Support clinical compliance and infection control protocols, ensuring safety and wellness in every aspect of care delivery. • Promote a resident-centered care model, quality care outcomes, and continuous improvement through active participation in clinical meetings and quality assurance initiatives. • Assist team members with clinical support and lead by example in professionalism, compassion, and skill.
