I am aware that I cannot make additional requests after submission.Additional Confirmations I have not requested more than one holiday off during my assignment.Additional Confirmations I am aware the the following shift structure is required: Combination of 12s and 8s to total 40 hours weekly. 12-hour shifts: 0700-1930. 8-hour shifts: 0700-1530Travel Contract Must travel at least 60.0 miles to receive a travel stipend.
