Role Description This is a full-time, on-site Licensed Practical Nurse (LPN) role located in Greenfield, IN. The LPN will provide direct patient care, administer prescribed medications, document patient information, and monitor residents' health status. Additional responsibilities include assisting in developing care plans, coordinating with other healthcare professionals, educating residents and their families on health topics, and ensuring adherence to all regulatory standards and facility protocols. Qualifications Valid and active LPN license in the state of IndianaSkills in patient care, medication administration, and health monitoringExperience with care planning and coordination of healthcare servicesAbility to maintain clear, accurate, and timely patient documentationKnowledge of health education principles and ability to communicate them to residents and familiesStrong teamwork and collaboration skills within a clinical settingAttention to detail and adherence to regulatory standardsPast experience in long-term care or assisted living facilities is a plusBLS (Basic Life Support) certification is beneficial
