With over 100 clinicians, our mission is to deliver high-quality, compassionate, and coordinated medical care to patients across Skilled Nursing Facilities, Nursing Homes, Long-Term Care, Assisted Living, and Independent Living settings.We integrate care across disciplines, working closely with Hospitalists, Specialists, Therapists, Case Managers, and Behavioral Health to ensure the best outcomes for our patients.Position OverviewWe are seeking Board-Certified or Board-Eligible Family Medicine or Internal Medicine Physicians for immediate openings North and South Carolina. The ideal candidate will provide compassionate, comprehensive care across post-acute and long-term care settings and collaborate closely with interdisciplinary teams.Key ResponsibilitiesProvide ongoing medical care and oversight to assigned patients in skilled nursing, subacute, assisted, and independent living settings.Create and maintain timely, appropriate, and confidential documentation in line with medical and billing standards.Possibly provide Medical Directorship service to one facility (that is included in the base compensation).
