Some patients/families are supported as well through virtual/phone based social work support either in place of in home visits or in combination with in home visits.Manages a Caseload Of Patients In Their Homeswork with an interdisciplinary care team of nurses, therapists and aidesconduct a comprehensive assessment of patient/family needslink individuals to the community resources they need to remain in their home or move to a higher level of care if necessaryOnce trained, enjoy the independence of starting and ending your day from home with a flexible work scheduleBe on the front-line of the transitions in health care from inpatient settings to the community – home health care is the future!Work schedule is flexible.RequirementsNYS Social Work Licensure, minimum of 1 year of relevant experience in home care, the medical field, work with seniors, etc. required. Eddy VNA provides a comprehensive orientation to home health care. Eddy VNRA provides a supportive work environment with access to social work supervision, in-depth computer training, assistance obtaining insurance authorizations, support staff who are only a phone call away, and more!Join us for a day and shadow an experienced Eddy VNRA social worker!Pay Range: $30.00 - $43.08Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates.
