Community Care Network is looking for a Manager – Shared Living to join our team!Community Care Network is comprised of Rutland Mental Health Services and Rutland Community Programs. Our mission is to enhance the well-being of our communities, individuals and families through a responsive, innovative and collaborative human serviceAbout The RoleThe Manager of Shared Living is responsible for overseeing the day-to-day operations, compliance, and quality of the Shared Living program while serving as the primary point of contact for Shared Living Providers. This role ensures providers are supported with clear expectations, training, and ongoing guidance, while also maintaining adherence to regulatory, documentation, and program requirements. It involves close coordination with clinical, case management, and operational teams to support placements, manage service changes, address concerns, and ensure consistent communication across all stakeholders.In addition, the Manager leads provider recruitment, onboarding, and retention efforts, monitors program quality through audits and incident review, and supports financial and operational processes such as provider payments, authorizations, and capacity planning. The role is focused on maintaining strong provider relationships, ensuring compliance and quality standards, and continuously improving outcomes for individuals served within the Shared Living program.ResponsibilitiesShared Living Provider OversightServe as the primary contact for Shared Living ProvidersEnsure Residential Inspections are completedMonitor provider performance and satisfactionSupport SLP’s with problem-solving and crisis coordinationEnsure providers understand expectations, policies, and compliance requirements Compliance and Documentation Monitor compliance with:DDS Medicaid Manual requirementsService documentation expectationsEVV/timekeeping requirements (if applicable)DOL and wage/hour concernsReview monthly summaries and provider documentation for quality and completenessCoordinate corrective action plans when neededCoordination with Clinical & Case Management Teams Work closely with care managers and program staff regarding:Variance requestsBehavioral or medical concernsService changesTransitions and placementsParticipate in team meetings and ISP-related planning as neededHelp ensure consistency in communication across teamsRecruitment and RetentionRecruit new Shared Living ProvidersConduct interviews, orientation, and onboardingDevelop retention strategies and provider supportsCoordinate respite coverage planningQuality AssuranceMonitor incidents and trends within Shared LivingConduct audits of files and documentationTrack outcomes and satisfactionIdentify program improvement opportunitiesFinancial & Operational Coordination Coordinate with payroll/billing regarding:Provider stipends/paymentsContracted supports concernsAuthorization changesMonitor vacancies and placement capacityAssist leadership with forecasting provider and staffing needsQualificationsOther duties as assigned.Bachelor’s degree in human services, social work, psychology, healthcare administration, or related field preferred; equivalent combination of education and experience considered3–5 years of experience in intellectual/developmental disabilities (I/DD), residential services, or community-based programsExperience in shared living, case management, quality assurance, or compliance strongly preferredKnowledge of Medicaid-funded services and applicable state/federal regulationsStrong understanding of residential service delivery and shared living modelsKnowledge of compliance, documentation standards, and quality assurance practicesAbility to build and maintain effective provider and stakeholder relationshipsStrong communication, coaching, and conflict resolution skillsDemonstrated ability to manage multiple priorities and maintain attention to detailAnalytical skills with the ability to identify trends, risks, and improvement opportunitiesCollaborative approach with cross-functional teams including clinical, billing, and operationsAbility to work independently while exercising sound judgment and decision-makingMust have a valid driver’s license, and reliable transportation with personal automobile liability insurance coverage at or above the levels mandated by the State of Vermont.*Successful completion of a criminal history background check after hire.Benefits Competitive Salary $63,000.00-$70,500.00 (compensation is typically dependent upon experience)Comprehensive Benefit Package401(k) 401(k) matching Dental insurance Employee assistance program Flexible schedule Flexible spending account Health insurance Health savings account Life insurance Paid time off Professional development assistance Retirement plan Tuition reimbursement Vision insurance Career Training and DevelopmentCollaborative and engaging team meetings with supervisionOur work is guided by our Core Values of Trust, Accountability, Respect, Cultural Competence, Person-Centered Care, and Continuous Learning and Growth.CCN is an Equal Opportunity Employer.
