Provide Medicare/Oasis and hospital-level PT care in patients' homes focusing on mobility, balance, fall prevention, and discharge readiness.Why Join Activa Home HealthProvide Medicare and acute-level, hospital-alternative care in patients' homesFlexible PRN schedulingWeekly payPaid Time Off (PTO)Paid orientation and program-specific onboardingOngoing clinical education and mentorshipCollaborative interdisciplinary teamMileage reimbursement (with signed agreement)Employee Assistance Program (EAP)Employee discount programsCareer advancement opportunities (PRN → Full-Time)401(k) retirement plan & Company-paid life insuranceMedical, dental, and vision insuranceTherapist appreciation and recognition programsJob ResponsibilitiesProvide skilled physical therapy services to patients receiving hospital-level care at home and for Medicare Skilled Patients.Perform comprehensive PT evaluations addressing: strength, balance, endurance, and functional mobility; gait deviations and fall risk; transfers, bed mobility, and stair negotiation; activity tolerance and cardiopulmonary response.Develop and implement individualized, evidence-based treatment plans aligned with acute recovery and discharge readiness.Address functional decline related to recent hospitalization, acute illness or injury, deconditioning and mobility loss.Educate patients and caregivers on safe mobility techniques and transfer strategies, fall prevention and home safety, use of assistive devices and mobility equipment.Monitor patient progress closely, adjust plans of care, and communicate changes promptly with the care team.Collaborate with physicians, nursing, OT, and care management to support safe transitions and reduce hospital readmissions.Complete timely, accurate documentation in compliance with Medicare, hospital-at-home, and agency standards.Qualifications RequiredActive Physical Therapist (PT) license in the state of FloridaDegree from an accredited Physical Therapy program (DPT preferred)Current CPR certificationValid driver's license and reliable transportationStrong clinical reasoning, communication, and time-management skillsSkilled in: gait training and balance rehabilitation, fall risk assessment and prevention, functional mobility and discharge planning, post-acute and medically complex patient care, comfortable practicing independently in a home-based acute care setting, familiarity with EMR documentation systemsAbout Activa Home HealthActiva Home Health delivers high-end, patient-centered care through innovative programs designed to meet patients where they are—including hospital-level care at home.
