Our Mission is to promote opportunities, which enhance the self-sufficiency of people to empower them to live and function independently.\n\nJob Description\n\nThe Healthy Start Case Manager provides client-centered case management by assessing participant needs, developing service plans, connecting families with medical and community resources, and coordinating care with providers.
