Ensuring the health and safety of the individual, including personal care. Providing education, training, counseling, and physical assistance to achieve ISP goals in functional academics, domestic skills, health and personal hygiene, social and leisure skills, communication skills, community mobility, self‐esteem, and money management. Communicating with family, friends, guardians, advocates, volunteers, and employers to facilitate consistency of programming and support. Developing, writing, and presenting annual ISPs and semiannual agreements in consultation with residential staff, Program Manager, and Director. Performing case management duties. Providing transportation to individuals as necessary. Scheduling and supervising staff.QualificationsExperience working with individuals with ID/DD. A bachelor's degree or associate degree in a related field with two years of experience, or an HS diploma and five years of related work experience in human services. Ability to pass fingerprint background check, CORI, DPPC and driving background checks. Valid driver's license required. Valid MAP certification preferred.Excellent verbal and written communication skills.
