This role blends hands-on patient care with leadership and program development , offering a strong pathway for professional growth.Key Responsibilities Clinical Leadership (Startup Phase) Serve as RN Case Manager (admissions, routine, and urgent visits) Provide direct hospice care in home, ALF, SNF, and hospital settings Participate in weekday on-call coverage with clinical support Leadership & Team Development Lead, mentor, and support RN, LVN, and hospice aide teams Ensure high-quality, patient-centered care delivery Provide real-time clinical guidance and field support Program & Quality Development Participate in interdisciplinary team (IDT) meetings Develop and improve clinical workflows and best practices Collaborate with leadership to expand hospice services Requirements Active Texas RN license required Strong background in hospice, home health, or acute care leadership Passion for end-of-life care and patient dignity Ability to work independently in field-based settings Apply Today ?
